What is a Primary Care Network?
A Primary Care Network (PCN) consists of groups of general practices working together, and in partnership with community, mental health, social care, pharmacy, hospital and voluntary services in their local area. PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home. Clinicians describe this as a change from reactively providing appointments to proactively caring for the people and communities they serve.
About Picton PCN
The Picton Network consists of six GP practices across the Picton Area:
- Edge Hill Health Centre
- Dunstan Village Group Practice
- Sefton Park Medical Centre
- Earle Road Medical Centre
- Picton Green Family Practice
- Green Lane Medical Centre.
Our Network Team work across these practices, with the aim of improving our patient’s wellbeing and addressing health inequalities in Picton.
